The Ohio State Chiropractic Association (OSCA) is pleased to announce the formation of our fully-insured medical and ancillary benefits program(s) for Chiropractors. Membership in the OSCA is more important than ever, and another tangible benefit to membership in the OSCA is the ability to access benefits through our plan with United Healthcare (UHC).


To obtain a quote you will simply need to complete and return the following information to the OSCA. If you are currently not a member, you will need to join the OSCA if you accept any of the plan options provided. Coverage will be available on December 1, 2016 (please do not terminate coverage until OSCA-UHC coverage has been verified for your group).


1. ALL Groups to complete: UHC Census  Great News!  Simply print and hand write the 2-pages to the best of your ability.  If we need more information, we will call you.  If your spouse works in your office, you may elect to apply individually (2 employees) or together in employee/spouse or family coveragePage 1  - Page 2


2. ALL Groups to provide: Copy of current group benefit plan and most recent group bill.

STOP - If your group has 5 or more employees covered on the current plan,you do not need to provide any additional information.


3. Groups with 2-4 Employees covered:OH UHC Employee Enrollment form to be completed by employees enrolling in the plan. Please do not email the employee application as it contains confidential information.


The OSCA will match the incoming fax applications to your emailed materials above. You may fax this to: OSCA Attn: Benefits, 614-229-5296 (fax)


The OSCA will see that all benefits options are quoted and provided for your convenience and consideration. Should you decide to enroll in the OSCA plan for December 1, 2016, a group employer application and premium binder check will be requested prior to the effective date of coverage.


Please take a few minutes to obtain your quote as soon as possible, but be sure to return these materials no later than October 30, 2016.
